Introduction to Health Assessment Ashgrove

At Ashgrove GP, we offer health assessment of older persons. It is an in-depth assessment of a patient aged 75 years and over. Our doctors provide a structured way of identifying health issues and conditions that are potentially preventable or amenable to interventions in order to improve health and/or quality of life. We offer this health assessment in order to identify any risk factors exhibited by people over 75s that may require further health management.

Our area, with an aging population, aims to assist patients’ medical and non-medical needs, from acute to chronic, through annual health assessments. It is widely accepted that a multidisciplinary approach to aging care best draws on the knowledge and skills of a range of health care professionals to help individuals achieve their highest levels of physical, functional and cognitive health.

Who is eligible to receive a health  assessment

You are eligible if you are aged 75 years or over and living in a community setting. You must be a regular patient of the surgery. Patients are only eligible to have one health assessment completed each year.

What happens during a health assessment?

Our practice nurse who will perform some simple tests and ask you some questions about your medical history.
This might include the following:
  • Vision check
  • Hearing check
  • Skin check
  • Urine sample
  • Blood Pressure
  • Medication review
  • Immunisation status
  • Weight
  • Height
  • Questions about your nutrition/diet
  • Questions about your social activities
  • Questions about your daily activities
